As we move into the 2024-25 respiratory virus season, we are reaching out about exciting programming for the fall and winter seasons. Here at Prism Health, we are passionate about the safety and well-being of your residents. We strongly believe a comprehensive approach of infection prevention, testing, vaccination and treatment is vital for preparing and responding to respiratory viruses within long term care facilities.  As we prepare for the upcoming months, our goal is to continue to provide seamless, effective and comprehensive solutions for your facility. Fall & Winter Vaccination

Prepare for respiratory virus season by protecting your residents. The prevalence of respiratory viruses increases during fall and winter months, making this a crucial time to be sure your residents are up to date for annual vaccinations such as COVID-19 and Influenza (flu). Vaccination remains a simple yet effective means of reducing serious illness, hospitalization and even death in our aging populations. Outbreak Testing

Allow Prism to collect specimens with our highly trained staff, freeing up valuable time for your current employees. All employees are trained following the IDPH COVID-19 outbreak protocol, defined as 2 or more individuals who are laboratory positive for SARS-CoV-2 by antigen or PCR. Testing is performed in our own laboratory, allowing Prism to control the quality and timeliness of testing results, providing better customer care and more efficient isolation of outbreak cases. Choose COVID-19 testing or triplex testing (COVID-19, Flu, RSV) to identify illnesses in your facility with specificity. Consult with a team of experienced medical personnel proficient in the IDPH COVID-19 outbreak testing protocol allowing your facility to remain compliant with current guidelines.

We understand timing is essential and to help with preparation we are including the steps to facilitate a smooth onboarding process. 

1. Complete the Prism Onboarding PDF

Attached to this email, you will find the Prism Onboarding PDF. Please complete all relevant fields and return the filled form by replying to the LTC STATeam at [email protected]. This document will provide us with the necessary information to service your facility effectively. Note: If you have worked with Prism previously, there is no need to fill this out again. Please just let us know if there have been any changes.

2. Submit Face Sheets for Residents

To ensure accurate record-keeping, we request that you provide face sheets for all residents. Please fax them to (833) 929-3778. As we get closer to a mobile clinic, we will work with your facility to make sure documents are up to date. Note: If you have worked with Prism previously, we will only need to update your current list with changes / new admissions etc.

3. Review and Complete Consents and Referral Templates

  • Vaccination Consent Forms: Attached are the necessary consent forms for your residents. We kindly request your cooperation in obtaining these consents and providing them as soon as possible. This will allow us to begin to schedule your vaccination event.

  • Outbreak Management Referral: If you experience a respiratory virus outbreak, please keep the attached template on file and give us a call at 888-977-4086. For each outbreak, we ask that you complete the highlighted areas and have your Director of Nursing or Medical Director sign it.

4. Staff Testing and Vaccination

Staff testing and vaccination is not included at this time. A fee schedule for staff testing is attached for your review. If you choose to include staff in your vaccination program, please notify us and have each staff member complete the consent form and return it along with the resident consents. Please note that we do not accept insurance for staff members. The signed form should be returned to confirm participation. Staff vaccinations and testing need to be approved on a case by case basis with corporate. 

5. Schedule and Insurance

Our team will work with you to establish a schedule for testing and vaccinations, adhering to the outbreak testing protocol developed by IDPH. Please note that we will collect and bill resident’s insurance information for our services. Prism Health only bills insurance and never the residents or your facility directly meaning we do not balance bill. All services are provided with no cost sharing.
